ANKYLOSING SPONDYLITIS – AN AYURVEDIC PRESPECTIVE

 Ankylosing Spondylitis. You think it’s hard to pronounce? Try living with it.

Ankylosing spondylitis is a chronic inflammatory disease with variable involvement of peripheral joints. AS is a form of spondyloarthritis, a chronic, inflammatory arthritis where immune mechanisms are thought to play a key role. It mainly affects joints in the spine and the sacroiliac joint in the pelvis and can cause eventual fusion of the spine. Ankylosing Spondylitis is a rheumatic disease and chronic inflammation of spine and the sarcoiliac joints, targeting people of young age group. It is a chronic disorder generally affecting the axial skeleton and causes extreme back pain and back stiffness that result in decreased in spinal mobility and hinder physical functioning. 
The treatment of ankylosing spondylitis typically involves the use of medications to reduce inflammation, suppress immunity to stop progression of the disease, physical therapy, and exercise. Medications decrease inflammation in the spine and other joints. Physical therapy and exercise help to improve posture, spine mobility, and lung capacity. 
Ayurveda offers excellent therapies for treating Ankylosing spondylitis. The treatment comprises of a series of purification procedures for Detoxification through world renowned Ayurveda Panchakarma therapies along with the administration of researched medicines internally. 
The main mechanisms involved in treating the conditions such as fever, back pain, swelling at various joints, stiffness in neck and back bone was explained. Different medicines used for the treatment of symptoms and their mechanism of action were explained properly. Strict diet restrictions along with life style modification are essential parts of the therapy. Following the diet as advised not only helps in recovering fast but also in prevents further worsening of the condition. 
The importance of diet in treating the condition was also explained. Ayurveda offers excellent therapies for treating Ankylosing spondylitis. The strength of Ayurveda in the area of spine and joint treatments is globally appreciated as it works on the root cause of an issue. Treatment is usually aggressive, long-term and requires regular and careful follow-ups. 
The treatment comprises of a series of purification procedures for Detoxification through world renowned Ayurveda Panchakarma therapies along with the administration of researched medicines internally. Panchakarma procedures requires around 60 - 90 days based on the severity & chronicity of the disease. Strict diet restrictions along with life style modification are essential parts of the therapy. Following the diet as advised not only helps in recovering fast but also in prevents further worsening of the condition.
ANKYLOSING SPONDYLITIS SYMPTOMS: The different signs and symptoms of Ankylosing Spondylitis are: 
• Chronic pain, inflammation and stiffness in the lower part of the spine.
• Ankylosing spondylitis is associated with an inflammation of eyes. 
• Feeling of fatigue.
• Lung fibrosis may occur.
• Functional mobility limited.
• Chest expansion problem.
• Loss of appetite. 
• Weight loss in the initial stage.
• Bowel inflammation.
• Joint pain.
• Neck pain.
• Heel pain
CAUSES OF ANKYLOSING SPONDYLITIS: Exact cause of ankylosing spondylitis is unknown, yet some factors are narrated here:
• Ankylosing spondylitis is likely to be genetically inherited. 
• A gene called HLA-B 27 is associated with ankylosing spondylitis.
• Prolong stressful condition may triggered it.
TESTS FOR ANKYLOSING SPONDYLITIS: The lists of diagnostic tests are:
• X-rays.
• Blood Tests (HLA-B 27 but the test is expensive).
• Medical history.
• Physical examination.
AYURVEDA LINE OF TREATMENT FOR ANKYLOSING SPONDYLOSIS: The objectives of Ayurvedic treatment for ankylosing spondylitis are to ease inflammation and rigidity in the spine and reducing inflammation in other affected organs. 
• Initially the patient is treated for the Fever (Jwara chikitsa), as fever is one of the main presentations. For treating the fever Amrithothara kwatha, Amavatariras tablet, and Amrutharishta will be advised. 
• When the fever subsides completely Lepa (external application of medicated paste) will be applied to the sacroiliac and other painful joints together with a heated mixture of kottamchukkadi choorna and dhanyamla. It will reduce the swelling in knee joints.
• If the patient suffers from skin rashes Grihadhoomadi lepa will be suggested.
• Internal medication will be changed to Indukantham , Marma gulika, Shaddharana choorna .
• While externally, Rooksha Pinda Sweda (sudation with medicated bolus) with Kolakulathadi choorna will be suggested for seven days.
• Purgation (Virechana) with gandharva eranda (Roots of Ricinus communis processed in castor oil.) on alternate days for one week to improve the appetite. 
• To treat morning stiffness Niruha Basthi (medicated enema) will be planned with modified Balaguduchyadi yoga mentioned for vata vikara. Basti (enema) will be administered according to kala basti format during which six niruhas (decoctions) and nine anuvasanas (oil) will be administered.The medicine for niruha basti (decoction enema) is prepared by mixing the drugs in a mortar with pestle, adding component medicines in the following order: makshika (honey), lavana (salt), sneha (oil), kalka (paste), and kwatha (decoction). It is administered on an empty stomach, using a conventional basti netra (syringe) and rexine encase as basti putaka (bag). Sahacharadi taila is used for anuvasana basti (oil enema), the dose being 60ml, administered on alternate days after food. 
• Simhanada guggulu, and Gandharva eranda, Ayaskrithi is administered for a period of 1 month, Mild spinal exercises were also advised to prevent occurrence of stiffness in due course of time.
• The extract of turmeric is effective in reducing the pain and inflammation of all types of arthritis and joint pains.
• Giloy is not only effective for ankylosing spondylitis but for osteoarthritis, rheumatoid arthritis too.
• Vitex negundo is extremely useful for ankylosing spondylitis.
• The analgesic and anti-inflammatory feature of Commiphora mukul, is instant.
• Dry Ginger is used to reduce inflamation.
• Ashwagandha is a known product for ankylosing spondylitis.
HOME REMEDY FOR ANKYLOSING SPONDYLITIS:
• Use ginger and black Pepper as it detoxifies your body. 
• 1 tsf of Turmeric + boil milk 25-30 minutes, lessen pain.
• Use juices like amla, aloe vera and other herbal one because all these are strengthen the immunity.
• Castor oil + Milk are useful for Ankylosing spondylitis.
• Use fibre so to avoid constipation as constipation may exaggerate the disease.
• Drinking water from a copper pan after keeping it overnight show good result for the disease.
YOGA ASAN FOR ANKYLOSING SPONDYLITIS: Yoga is a mind- body -soul therapy of alternative system of medicine, where breathing exercise is popular for Ankylosing Spondylitis. Yoga therapy helps in minimizing lower back pain, softens the spinal stiffness and vigor the spinal muscles, enhancing functional capacity and improves spinal flexibility.
• Aasanas like Padahastasana, Ardha chakrasana, Ardhakatichakrasana, Pawanamuktasana, Bhujangasana, Shalabhasana, Dhanurasana, Paschimottanasana, Vakrasana are beneficial for the diseases.
• Pranayamas such as Nadishodana, Chandrabhedi Sheethali and Bramari pranayama is useful for the disease.
• Kapalbhati is also effective.
PATHYA AAHAR & VIHAR (COMBATIBLE DIET & REGIMES): The patient's diet will be restricted to rice gruel, yavagu, processed with panchakola churna, with cooked green leafy vegetables with minimal salt and oil. Mudga yusha (green gram soup) was allowed. If the appetite improves then the patient will be allowed to take easily digestible steamed food items. After the Swedana (sudation), the patient fell good relief from the low back pain and morning stiffness.
Early detection, proper and suitable treatment at the right time may help. Some regimes to be followed are
• Keep yourself away from cold and dampness.
• keep yourself warm.
• Prevent injuries.
• Eat a balanced diet.
• Vitamin C & A, and consumption of pears, apples, berries, pawpaws etc. is recommended as these bolster the immune system.
• Essential supplements like omega-3 oils, calcium, magnesium and sodium should given to the patients.
CONCLUSIONS: As there is no permanent treatment for Ankylosing Spondylitis life style modifications and dietary modifications should be given proper importance to improve the quality of life. Research should be continued to prevent the occurrence, to control the progression of disease and to treat acute condition of Ankylosing spondylitis. Scientists should concentrate on the alternative systems of medicines such as Naturopathy and ayurvedam to improve the condition. Both allopathic and ayurvedic treatments should be considered together for better control of the disease.
